From the ORB: SUPERVISION TERMINATION: The Plaza at Pembroke Park Assisted Living Facility, 3535 SW 52nd Ave., Pembroke Park, FL 33023

March 17, 2017 - From the ORB:

Effective immediately, the Plaza Assisted Living Facility food service program is no longer being supervised by the ORB. The facility has been sold, and the new owners have chosen to discontinue the ORB's certification. For further information, please call 775-863-8871.
