The following news is from the FDA posted on May 21, 2015.

The FDA tested 94 dark chocolate bars for the presence of milk. Six of the bars listed milk as an ingredient. The FDA found that 51 of the remaining 88 bars contained milk. The FDA found milk in 61% of the bars that they tested. The FDA found 3 out of 4 dark chocolate products with a warning notice contained milk. The also FDA found that 25% of dark chocolate products labeled only “vegan” were found to contain milk.
"Most dark chocolate is produced on equipment that is also used to produce milk chocolate. In these cases, it is possible that traces of milk may inadvertently wind up in the dark chocolate." In addition (the FDA did not mention this) powdered milk from the air can get into chocolate.

Ed. note: People with severe dairy allergies need to know that chocolates labeled as Pareve are halchically pareve but may contain tiny amounts of milk.
